Camp Waiver & Release
Every person attending camp must complete and submit a Camp Waiver & Release. Parents of minors should complete the form. Group leaders and adult sponsors will complete their own forms.

You will need your registration ID #. You can find that number on your registration confirmation email or the January Monthly Update Email from the FUGE Camps office.

Once a form is completed and submitted, you will receive an email. Please check off names as they are submitted so you will know who has not completed the form to follow up. These forms must all be completed TWO WEEKS before you arrive to camp.
You do not need to give FUGE Camps a printed copy, but we are encouraging you to print one copy of your camp participant forms and insurance cards and bring them with you to camp.
Church Agreement Form
This form ensures that your adults have been properly screened. Every Group Leader and Adult Sponsor at camp must have had a clear criminal background check before coming to camp. We do not need copies of the actual background check. You are also required to cross check every adult sponsor with the National Sex Offender Website.

Certificate of Insurance
This form also addresses liability and insurance. Lifeway is asking for a Certificate of Insurance (COI) for a Commercial General Liability policy from each church attending FUGE Camps for 2023. This type of liability insurance covers the church from any claims made against your group due to your negligence while attending camp.

Child protection Training – New for 2023
All adults attending camp must complete our Child Protection training module. This consists of 5 videos to watch and a test to take at the end. Once this is complete, print your completion certificate and give it to your group leader by the due date he/she specifies. These certificates will be collected at Registration on the day you arrive at camp.

Background Check
Every adult sponsor coming to camp must have had a background check within the last year. One low cost option to run these background checks is found below. You do not have to use this option for your background checks, if you have another company you would prefer to use.

MFUGE Transportation Policy (MFUGE only)
Group leaders attending MFUGE must read and sign the MFUGE Transportation Policy, agreeing to bring adequate transportation to camp.
Sponsor’s Children at Camp Agreement (If Applicable)
Parents bringing their children to camp must complete the Sponsor’s Children at Camp form and email it to the camp email address before they arrive at camp and abide by those guidelines

Child Protection Training
Glorieta and Texas Locations: (Austin College, Southwestern Seminary, Houston Baptist University)
We must operate under guidelines set forth by The Texas Department of State Health Services, which is the licensing agent for camps. These laws require any adult who has contact with children/youth at camp to be screened for criminal and sex offender records.

Pennsylvania Location: (Eastern University)
We must operate under guidelines set forth by The Pennsylvania Child Protective Services (http://keepkidssafe.pa.gov/resources/clearances/index.htm) for FUGE Camps held in Philadelphia.
These laws require any adult who has contact with children/youth at camp to be do the following:
Pennsylvania Child Abuse History Clearance, Pennsylvania State Police Criminal Record Check, Federal Bureau of Investigations (FBI) Criminal History Background Check.
