Camp Waiver & Release
Please fill out the FUGE Camps Camp Waiver & Release below. Every person attending camp must complete and submit a Camp Waiver & Release. Parents of minors should complete the form. Group leaders and adult sponsors will complete their own forms.
*If your group was registered after February 8, 2023, please wait at least 7 days from the time you register for camp to complete the Camp Waiver & Release.
NOTE: The group leader of your group will need to provide you with the Registration ID # in order for you to complete this release.
